Responsible for providing skilled nursing services to patients in their homes under the direction of a physician. Must assess, plan, implement, document, evaluate and teach patients’ needs and care. As a Home Health Nurse, you will see about 5-6 patients each day and have a case load of approximately 20 patients at a time. This position offers flexibility and needs you to be flexible in return. It will require a lot of driving, and you can take advantage of our Fleet Car Program after orientation.
Provides professional skilled nursing to a patient population experiencing a wide variety of illnesses or recovery from injury. Develops a plan of care based on nursing assessment, physician orders; works with patients and families to develop realistic and measurable goals. Provides pertinent/accurate education and instruction to patients/family members/caregivers. Validates understanding of education using a variety of technique i.e.: verbal feedback, return demonstration. Evaluates effectiveness of plan of care, updates as necessary. Anticipates and responds to changes in patient clinical status, communicating with physician to obtain orders and other members of the health care team in a timely manner. Identifies need for Medical Social worker, physical therapy, speech therapy, occupational therapy and nurse assistant services to ensure optimal outcomes. Is attentive to the home environment, recognizing potential safety concerns, possible signs and symptoms of abuse and neglect and reports such findings to appropriate co-workers, division of family services as necessary. Collaborates with patient/family/caregiver and health care team members to encourage independent care and plan for safe discharge from home care services when appropriate.
Performs nursing procedures and treatments with proper technique, in compliance with standard nursing policy and procedure, within the scope of the nurse practice act. Seeks and maintains knowledge of variety of disease processes , medications and treatment of injury individual to patient caseload.
Documentation is accurate and completed within designated timeframe. Documentation reflects knowledge of Oasis/Medicare/Medicaid regulatory guidelines. Maintains flexibility with daily schedule and demonstrates ability to prioritize.
Completes annual Mercy Competencies within designated timeframe. Assumes responsibility for continuing education to maintain current knowledge base. Participates in Quality Improvement activities, skill competency education and review. Collaborates with co-workers, including the marketing team to improve Mercy Signature Service and ensure provision of excellent care. Verbal and Written communication with co-workers, patients and families is provided in a professional and respectful manner at all times. Maintains a positive attitude while working within a challenging healthcare environment. Participates in on call, weekend and holiday rotation. Compliant with Mercy Attendance Policy.
