Responsibilities and Qualifications

Provides in home physical therapy services to patients with neuro and/or musculoskeletal disorders, under the direction of a physician. Performs duties and responsibilities in a manner consistent with our mission, values, and mercy service standards.

 

Schedules, evaluates, plans and administers patient treatment programs according to the patients physical, functional and self-care abilities. Involves directing and assisting patients in passive and active exercises, muscle re-education, gait and functional training. Administer therapy modalities as prescribed by physician. Monitors plan for effectiveness.

Consults with Physicians, nurses, patient, caregiver and other health care professionals regarding coordination of treatment, goal development and response to treatment. Involves review of initial goals established at the beginning of treatment. Reports on patient at patient care conferences. Identifies need for other disciplines.

Re-evaluates and revises treatment plan when appropriate using standard procedures. Orders equipment as needed. Functions within established policies, procedures and guidelines of the American Physical Therapy Association, and agency policies and procedures.

Educates patient and/or family regarding therapy program and safety. Fits, evaluates and adjusts adaptive devices. Prepares patient/family for discharge.

Documents accurately and timely the patients treatment, care, evaluation, and progress, and communicates with other staff members.

Supervises PTA and Aide as applicable. May orient, instruct and direct work activity.

Participates in Quality Improvement activities, inservices, patient care conferences, workshops and seminars. Assumes responsibility for continuing education and maintaining current knowledge.

Assess home situation for possible patient abuse.

Will assist with orientation and training of other staff members.

Participates in weekend & holiday schedule.

 

Qualifications:

  • Education: Graduate of an accredited physical therapy program.
  • Licensure: Current state license. Valid drivers license.
  • Certifications: CPR certification.
  • Other: Employee is frequently to continuously alone with patient and must be able to handle emergencies without assistance. Employee must be able to lift or assist falling or fallen patient; must be able to perform CPR alone, frequently for long periods of time without relief. Errors in therapeutic activities and exercises could cause serious injury to patients. Individual homes may present poor lighting situations, exposure to pets and other allergens, pest infestations and poor cleanliness. Employee may be exposed to irate and abusive patients and families. Employee may be required to drive at night and/or in hazardous conditions; must be able to follow written or verbal instructions and read a map. Employee must independently schedule patient visits in timely and efficient manner; checking in with office daily to update schedule. Employee is assigned weekend and on-call rotation. Employee has regular contact with agency staff members, physicians, hospital staff, and general public. Employee regularly handles confidential information.
  • Preferred Experience: Two years experience in acute care or home care preferred.
