Job Description Responsibilities: Responsibilities
Supervises daily operations of agency in an effective and efficient manner to meet professional, organizational and productivity standards.
1.1 Monitors clinicians’ workloads and assists them in making adjustments to their schedule in order to accommodate case loads and to meet productivity standards.
1.2 Works with case managers to assign patients based on acuity, appropriate skill level, and appropriate geographical area.
1.3 Schedules patient visits with appropriate attention to productivity standards and budget requirements.
1.4 Monitors equipment needs of clinicians and patients to facilitate procurement.
1.5 Reviews reports on productivity with the Clinical Nurse Manager daily.
1.6 Works to maintain and grow relationships with physicians and care managers to increase referral base.
1.7 Participates on committees as assigned.
1.8 Assures daily operations consistently meet the departmental policy and procedure standards and those of the various accrediting bodies.
1.9 Participates in Administrative on-call rotation.
1.10 Contributes to the preparation of the annual operating/capital budget.
Demonstrates effective personnel management skills
2.1 Establishes measurable goals, monitors performance, and provides feedback to co-workers.
2.2 Completes annual performance evaluations as assigned.
2.3 Participates in recruiting, interviewing, and retaining co-workers.
2.4 Assists with orienting new co-workers.
2.5 Encourages co-worker participation in agency activities, committees, and in-services.
2.6 Participates in appropriate disciplinary actions as needed.
Ensures that organizational and professional standards are met in the home setting.
3.1 Monitors clinical documentation to determine accuracy and that care is done in accordance with the written plan of care and compliance with agency policy.
3.2 Monitors documentation of current supervisory visits of the nurse aide every two weeks.
3.3 Conducts supervisory visits annually on all clinicians and assistants, educating co-workers as needed. Provide or arrange direct clinical supervision by discipline.
3.4 Conducts regular patient care conference and coordinates patient care with all patient care providers.
3.5 Helps to facilitate weekly case conference meetings.
3.6. Ensures all regulatory forms are completed by the clinicians in a timely manner.
3.7 performs monthly auditing of medical records according to policy. Facilitates process improvement as needed.
Works as a liaison between acute care, hospice, and other outside organizations to ensure patient’s needs are met in a timely and professional manner.
4.1 Works with care management and intake to ensure that patients that need home health are accepted in a timely and professional manner.
4.2 Keeps intake informed and updated on schedule changes and/or issues that would affect accepting referrals.
Participates in department educational and promotional activities. Identifies educational needs for the department.
5.1 Plans and schedules in-service programs throughout the year.
5.2 Participates in choosing and developing agency competencies annually.
5.3 Performs miscellaneous duties as requested by the manager/director.
As staffing needs dictate, provides skilled nursing assessments, interventions, and evaluations to patients in the home setting based on professional standards of care.
6.1 Coordinates all disciplines directly involved in the care of the patient.
6.2 Documents assessment of patient’s condition and status per policy.
6.3 Records data relevant to the patient’s care within 24 hours of visit.
6.4 Assesses, evaluates, and coordinates need for other disciplines as ordered in the plan of care.
6.5 Meets all regulatory time-lines and documentation standards required.
Demonstrates effective and accurate verbal/written communications and maintains computer competency.
7.1 Creates a positive climate for communications
7.2 Provides timely, accurate, concise, and appropriate communication.
7.3 Follows established channels for addressing issues, concerns and problems.
7.4 Demonstrates support for administrative and ministry decisions.
Performs other job duties as assigned.
