The Transitional Care Unit provides care to patients age 18 and older who do not require intensive care but require a higher level of care/monitoring than a general medical/surgical unit or the progressive care unit can provide.  The TCU is designated as a Level II care area. 


General Description:

The Transitional Care Unit (also known in Mercy as the Medical Surgical Intermediate Unit) is an six bed, interdisciplinary unit that is located on the first floor next to and in the same area of the Medical Surgical Intensive Care Unit.  All of the patient rooms are private. 


Scope of Care:  The unit exists to provide comprehensive, interdisciplinary care to adult medical-surgical patients who require intermediate level of care.  We care for a spiritual, ethnic and culturally diverse population.  Our interdisciplinary team works collaboratively in assessing and developing a unique, evidenced-based plan of care for patients and their family.  We are dedicated to the care, rehabilitation, and education of the patient and family.  Patients include but are not limited to:

  • Respiratory illness
  • Medically stable ventilator patients for weaning and chronic care
  • Cardiac/Cardiovascular illness
  • Endocrine illness
  • Neurological illness
  • Post-operative status
  • Patients who require a higher level of care/monitoring than general nursing units, but are stable and non-critical.


Position Qualifications:

  • We require a nursing degree from an accredited school.
  • You’ll also need a current RN license in our state.
  • Basic life support training should be completed in 90 days with advanced training required at certain intervals.
  • Licensure: Meets the criteria to be eligible for licensure in state of Missouri or currently licensed in the state of Missouri If working on the following areas additional requirements are as stated: • ED, PAR, ASC, GI Lab, ICU – ACLS required within first year
