Responsibilities and Qualifications

Perform initial assessment for each patient upon receiving referral according to departmental policies and procedures and standards of care.


1.1 Perform initial patient evaluation using appropriate objective tests and evaluations
with no errors.
1.2 Inquire as to patient's (and/or care giver's) perceived goals.
1.3 Interpret objective tests, and with the patient's (and/or care giver's) input,
establishes functional treatment goals with defined time frames.
1.4 Courteously inform the patient (and/or care givers) of modes to achieve perceived
goals.
1.5 Collaborate with other professionals as appropriate should the patient require an
interdisciplinary plan of care.
1.6 Document initial evaluation/plan of care with clear and measurable treatment goals
within 24 hours. Establish medical necessity for treatment and procedures to be employed for attaining goals of treatment.
1.7 Complete coding on initial evaluation.
Administer appropriate modalities, exercise regimens, and/or treatment procedures according to departmental policies and procedures and standards of care.


2.1 Develop effective age-appropriate treatment plans for patient populations.
2.2 Courteously explain to patient (and/or care giver) treatment procedures before
administering.
2.3 Perform skilled procedures in a professional and courteous manner.
2.4 Analyze behavioral responses and, if necessary, modify treatment plan.
2.5 Utilize appropriate safety techniques and infection control methods.
2.6 Collaborate with other professionals as appropriate for the treatment of all
interdisciplinary patients as described in their respective plan of care and according to
departmental standards of care.
2.7 Reassess and update goals; modify treatment as indicated to facilitate best quality
of care for each individual patient.
2.8 Communicate with referring physician as required.
2.9 Score 85% with individual quarterly chart audit results.
2.10 Document daily any changes in status or progress with treatment.
Prepare patient for discharge according to departmental policies and procedures and standards of care.


3.1 Discharge programs are to be made 1 visit prior to last treatment session.
3.2 Educate patient and, if necessary, family member(s) or care giver as to home
exercise instructions.
3.3 Collaborate with other professionals as appropriate for all interdisciplinary patients
who are soon to be discharged.
3.4 Document discharge notes according to documentation standard and time guidelines.
Demonstrate knowledge of special needs and behaviors of specific age groups to include neonatal, pediatric, adolescent, adult and geriatric.


4.1 Demonstrate knowledge of growth and development.
4.2 Demonstrate ability to assess/interpret age specific data.
4.3 Demonstrate ability to provide age specific data to other care givers.
4.4 Demonstrate ability to interpret age specific responses to treatment.
4.5 Demonstrate ability to involve family and/or significant others in decision-making
related to plan of care.
4.6 Demonstrate ability to provide care based upon physical, psychosocial, cultural,
safety and other age related factors.
Operational responsibilities.


5.1 Manage patient load according to productivity standard(s) of the department. Assist in management of weekly departmental productivity target by facilitating a full work schedule. Monitor individual caseload and make necessary changes in worked hours to facilitate patient flow and productivity.
5.2 Assure all appropriate charges are recorded and entered by the end of the day.
5.3 Assure daily operations consistently meet the department's policies and
procedures, standards of care, and those of various accrediting bodies.
5.4 Continuously provide oversight and reporting to Corporate Compliance Officials of
any potential irregularities indicating potential fraud and abuse activities.
5.5 Maximize the use of techs, registration specialists, therapy assistants, staff and
other resources for optimal efficiency while still maintaining high standards of care.
Promote continuous quality improvement, education, and a positive atmosphere.


6.1 Use creative problem solving skills and lead by example with a positive attitude.
6.2 Duties may include supervising Physical Therapy students according to Mercy
Health Center and School policies and procedures.
6.3 Assures department and personal area maintain a clean and safe work
environment.
6.4 Continuously seek and implement quality improvements while sharing results and
ideas with staff, peers, and manager.
6.5 Attend 100% of hospital mandatory meetings with at least 80% of staff meetings.
6.6 Maintain current CPR certification and state licensure.
6.7 Perform miscellaneous duties observed as necessary and directed by supervisor.
Perform daily and interim documentation according to departmental policies and
procedures.


7.1 Document daily any changes in status or progress with treatment procedures as
relative to defined treatment goals. Reassess and update goals as indicated.
7.2 Analyze physical and behavior responses to treatment, and if necessary modify
the Plan of Care/treatment plan.
7.3 Daily documentation to include time frames for each procedure, and demonstrate
medical necessity for procedures utilized in treatment.
7.4 Identify or create written home exercise instruction(s), outlining repetitions, frequency and precautions described in their respective Plan of Care, and according to departmental Standards of Care.
7.5 Document timely Interim Plan of Care documents. Note to include patient's
response to treatment procedures with consideration to pre-established goals. Define
new goals for treatment with specifics of frequency and duration, number of visits previously attended, and new certification dates for continued treatment.
7.6 Attend team meetings as indicated with completion of team documentation prior to
each meeting.
7.7 Address treatment goals within the daily documentation as indicated. State
hindrances to goal achievement.
7.8 Maintain the medical summary document of each patient.

 

Qualifications:

Education: Minimum of a Bachelor of Science in Physical Therapy.
Licensure: Current Oklahoma license of Physical Therapy.
Certifications: Current CPR certification.

Preferred Experience: Six months to one year of experience caring for patients of all ages is preferred.
