Overview
The Nurse Navigator manages the operations of the patient care unit on a particular shift. They are leaders who provide support, mentorship, and guidance to those who provide direct patient care. The Nurse Navigator will coordinate the delivery of patient care by determining staffing needs and making patient assignments by choosing the right assignment for the right person. The Nurse Navigator will ensure that all patients receive timely, safe, effective and high quality care. They coordinate the patients plan of care with the bedside nurse. The Nurse Navigator will set the expectations regarding professional conduct and serves as a resource for the patient care unit. The Nurse Navigator will hold direct and indirect care providers accountable for their professional responsibilities and adherence to regulatory guidelines. Responsible for management for the assigned inpatient Care Management population. This position is designed to facilitate an effective process of the new care model; supporting quality patient care, safety and financial components; promoting integration of a seamless care model; coordinating patient discharge planning for post acute services, length of stay (LOS) management and utilization management; collaborating to include coordination as evidenced by metrics, optimizing performance and adoption of best practice. Responsible for pre-op education, daily physician rounding while the patient is in the hospital, and patient discharge disposition. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.

 

 

Position Qualifications:

  • We require a nursing degree from an accredited school.
  • You’ll also need a current RN license in our state.
  • Experience: 1 year acute care hospital setting.
  • Certifications: BLS (CPR) at hire date, Minimum Required.
