Mercy Hospital JFK Clinic serves the health needs of individuals who are uninsured or underinsured. We offer 3 healthcare services through an OBGYN Residency Program, an Internal Medicine Residency Program, and Pediatric Department. The Clinic is open Monday-Friday, 8a-4:30p.

 

Responsibilities and Qualifications

The Internal Medicine Social Worker will provide social work services for adult patients and their families. This includes working with physicians, nurses and clinic staff to enhance each patient’s health and psychosocial functioning. The social worker will exhibit understanding of the multi-faceted needs of a diverse and underserved adult population. This is a part-time, job-share position and is not benefit (paid time-off or health insurance) eligible.

Responsibilities:

  • Actively supports clinic staff to improve patient outcomes.
  • Assists team and patient in the clarification and implementation of goals and plans of care.
  • Conducts psychosocial assessments and plans strategies for intervention.
  • Provides support and crisis intervention related to patient’s medical condition, mental health needs and/or family dynamics.
  • Facilitates referrals to community resources including home health care and durable medical equipment.
  • Performs duties and responsibilities in a manner consistent with Mercy’s mission, values and Service Standards.

 

Qualifications:

  • Applicant must have a Master’s Degree in Social Work or related field.
  • Licensed Clinical Social Worker or eligibility for licensure preferred.
  • 2 years experience in a healthcare, clinic, or mental health setting.
  • Applicant must be sensitive to the needs of low income/low opportunity community.
  • Applicant must be able to work independently and as a member of a team.
  • Applicant should have a strong understanding of Microsoft Office. Previous experience with EPIC software or other electronic health record preferred.
  • Applicant must display flexibility, sound judgment and outstanding communications skills.
