Responsibilities and Qualifications

Responsibilities
Has knowledge and understanding of daily assignments and unit activities.
1.1 Takes assignment from charge nurse.
1.2 Answers patients' call lights in a timely manner to determine needs and responds appropriately by fulfilling needs.
1.3 Communicates with CNA's and NA's from preceding shift concerning patient needs.
1.4 Organizes and prioritizes patient care.
1.5 Performs designated tasks and completes delegated assignments.
1.7 Assists in admission transfer and discharge procedures.
1.8 Performs hourly rounding in conjunction with the nursing staff

Performs routine tasks and simple treatments under the direction of a registered nurse.
2.1 Takes and records temperature, pulse, respirations, blood pressure as directed by the licensed nurse. Reports any significant findings to the licensed nurse
2.2 Obtains weights on patients as ordered.
2.3 Performs fingerstick blood sugars and records results in medical record. Reports abnormal results (less than 80 or greater than 120) to the nurse.
2.4 Empties wound suction/drainage devices at 0600, 1400, 2200 and prn; records amount in the medical record.
2.5 Assists in managing equipment specific to the nursing unit such as ice packs, heating pads, intermittent pneumatic compression devices and CPMs to excude equipment related to medications and enteral feedings.
2.6 Prepares and assists patients with Sitz baths as ordered.
2.7 Retrieve specimens, label them correctly and delivers them to the Unit Secretary.
2.8 Apply and removes restraints according to the current Mercy Health Center Policy. Record in the medical record all activities and observations according to the Restraint Policy.
2.9 Performs bladderscans and record finding according to policy.
2.10 Changes telemetry leads daily.
2.11 Turns and encourages deep breathing and coughing by post-operative patients every two hours.

Provides personal hygiene and comfort measures to patients as directed.
3.1 Provides complete/partial baths or showers to include oral hygiene.
3.2 Records hygiene and comfort measures in the medical record.
3.3 Performs catheter/perineal care every shift and documents this activity in the medical record
3.4 Assists with bedpan/urinal, bedside commode, or to the bathroom as needed.
3.5 Changes and/or cleans ostomy bags as needed records any output in the medical record.

Assists in activity.
4.1 Maintains proper body alignment and activity by turning non-ambulatory patients every two hours and documenting positionin the medical record.
4.2 Assists patient to chair or ambulates as ordered.

Assists in the maintenance of adequate nutrition/hydration.
5.1 Provides fresh ice water at least once per shift.
5.2 Assists patients with meals, feeding if necessary.
5.3 Records type of diet and percentage of food eaten on the graphic record.
5.4 Correctly follows patients' prescribed diet.

Demonstrates effective and accurate verbal, and written communication.
6.1 Promptly reports all signs and symptoms indicating any changes in the patient's condition to the licensed nurse immediately (complaints of pain, response to surroundings, changes in alertness, unusual skin redness or tears, etc.)
6.2 Reports to charge nurse before leaving assigned area.
6.3 Documents all care provided in the medical record.
6.4 Communication is timely, accurate, concise, complete, and appropriate.
6.5 Creates a positive climate for communications.

Maintains a clean and safe environment for patient and self by:
7.1 Emptying and properly bagging linen and trash and storing it according to hospital policy.
7.2 Cleaning and proper storage of bed side commodes, overbed tables.
7.3 Removing unnecessary equipment from room.
7.4 Straightening/changing linens daily and as needed.
7.5 Uses proper hand washing techniques and uses standard precautions at all times.
7.6 Complies with isolation procedures.
7.7 Keeps bed at low position and call light within patient's reach.

Demonstrates knowledge of special needs and behaviors of the type of patients that are typically assigned to the unit in which they work
8.1 Demonstrates ability to provide care based upon physical, psycho/social, cultural, safety, and other age related factors as appropriate for their patients.
8.2 Demonstrates the ability to obtain and interpret information as appropriate for the patient.

Promotes professional and organizational development by completing the following:
9.1 Attends mandatory in-services.
9.2 Maintains current CPR certification.
9.3 Assists with orientation of newly employed personnel and NA students.
9.4 Attends 75% of the unit's staff meetings.
9.5 Maintains current Fingerstick Blood Sugar certification by performing high and low glucose controls annually.

Provides unit support activities as necessary.
10.1 Maintain a clean, neat and safe environment in the Nurse's Station and the Galley.
10.2 Performs clerical duties as assigned by the licensed nurse.
10.3 Transports supplies from clean and dirty holding and/or supply rooms as needed.
10.4 Participates in unit process improvement activities.  

Qualifications:

Minimum Qualifications


Education: Must have High School diploma or GED.

 

Certifications: BLS required. Required certifications must be current before assigned to care for patients independently.

 

Other: Must be familiar with procedures involved in patient tasks. Completes orientation and all unit based education through Educational Resources within 3 months of employment.

 

Preferred Qualifications

 

Preferred Education: A nursing student that has obtained certification letter form a faculty member that verifies completion of at least the first two semesters of an approved nursing school; or Graduate of a nursing program who is waiting to take or retake state boards, or

Preferred Certifications: Currently a Certified Nurse Assistant or an EMT in the state of Oklahoma.
