The chaplain is responsible for the provision of spiritual care services to patients/families/ caregivers according to their desires and beliefs, either directly or through coordination of care with other spiritual counselors. Performs duties and responsibilities in a manner consistent with our Mission, Values, and Mercy Service Standards.
Assures spiritual assessment of patient/families/caregivers and appropriate services are rendered in a timely manner.
Provides direct spiritual support and/or counseling to patients/families/caregivers in keeping with patient/family/caregiver beliefs.
Attends and participates in interdisciplinary team meetings.
Documents accurately and submits documentation in accordance with Mercy Hospice policies.
Serves as liaison and support to community chaplains and spiritual counselors.
Participates in Performance Improvement activities.
Provides bereavement follow-up services as defined by Mercy Hospice.
Provides or makes arrangements for funeral or memorial services for patients as requested and to help meet the needs of bereaved families, personnel and volunteers working with the Hospice team..
Provides consultation, education and support to the interdisciplinary team on spiritual care.
Participates in the Admission Process. Admits patients into Mercy Hospice informing them of the services available, their rights and responsibilities as well as those of the organization, and any financial responsibility.
Follows the established uniform policy. Presents a professional appearance, maintains a positive public image and a professional work environment and wears their ID badge at all times.
Participates in required afterhours coverage as assigned. Bachelor’s degree in counseling or related field and specific end of life training per state regulations within 90 days of hiring. OR 1 unit of Clinical Pastoral Education. Bachelor’s degree in counseling or related field and specific end of life training per state regulations within 90 days of hiring. OR 1 unit of Clinical Pastoral Education.

Experience: Clinical experience working with patient/families dealing with life-threatening and chronic illness, grief and loss, individual, family and group therapy.
Other: Ability to be flexible, open, sensitive and ecumenical; ability to accept different lifestyles, cultures beliefs and values. Good oral and written communication skills; good organizational skills. Ability to work as member of interdisciplinary team.
Preferred Other: Basic computer skills as well as a working knowledge of Windows, Word and Outlook.
