    Responsibilities and Qualifications

    The purpose of the Medication Reconciliation Technicians is to review, clarify, document and verify with the patient if possible, their Prior to Admission medications within 24 hours of admission to the hospital. The Medication Reconciliation Technician will utilize the patients pharmacy, family, caregiver or prior office records if needed to further investigate and verify medications on, to be added or to be removed from the PTA medication list. Once the PTA list is completed a Progress Note will be sent to the Attending Physician to alert them of any changes, discrepancies or medications needing removed. The technician will keep records of all work done for medication reconciliation performance monitoring. Performs duties as assigned.

     

    Qualifications:

    • High School diploma or Equivalent.
    • Actively registered, in good standing, with the Missouri Board of Pharmacy as a Pharmacy Technician or willing to submit application for registration before the first day of work.
