Part-Time 28 Hours per Week
Provides in home physical therapy services to patients that have neuro and/or musculoskeletal disorders. Works under direction and supervision of a licensed Physical Therapist. Performs duties and responsibilities in a manner consistent with our mission, values, and mercy service standards.
Provides patient care, which includes providing treatment according to plan of care established by registered Physical Therapist, which involves directing and assisting patients in passive and active exercises, muscle re-education, gait and functional training. Monitors plan for effectiveness. Functions within established policies, procedures and guidelines of the American Physical Therapy Association, and Homecare policies and procedures.
Consults with Physician, Physical Therapist, nurse, patient, caregiver and other health care professionals regarding coordination of treatment, goal development and response to treatment program. Involves review of initial goals established at the beginning of treatment. Reports on the above to the Physical Therapist and at patient care conferences. Identifies need for other disciplines.
Educates patient and/or family regarding exercise, exercise technique and safety. Fits, evaluates and adjusts adaptive devices.
Assists Physical Therapist in preparing patient and/or family for discharge. Assists with obtaining equipment.
Documents accurately and timely patient treatments, progress, observations, and communications with Physical Therapist and other staff members.
Participates in Quality Improvement activities, inservices, patient care conferences, workshops and seminars. Assumes responsibility for continuing education and maintaining current knowledge.
Assess home situation for possible patient abuse.
Assist with orientation and training of other staff members.
