    Responsible for providing skilled nursing services to patients in their homes under the direction of a physician. Must assess, plan, implement, document, evaluate outcomes.


    Provides professional skilled nursing care to patients in the home under the direction of a physician. Develops/updates an individual nursing plan of care based on nursing assessment, physician orders; works with the patient/family to establish realistic and measurable goals. Evaluates patient's response to treatment and reports pertinent information to physician and other health care team members accurately and in a timely manner. Instructs patient/family/caregivers on all aspects of care, validates understanding of instruction using a variety of technique ie: verbal feedback , return demonstration. Consults physician and obtains orders as appropriate to bring in additional health care team members such as Medical Social Worker, Physical Therapy, Occupational Therapy, Speech Therapy or Nurse Assistant services to ensure optimal patient outcomes. Is attentive to patient's home environment, assessing safety risks, signs/symptoms of neglect and/or abuse and reports concerns to health care team, division of family services as appropriate. Encourages patient/family's independence with care, collaborates with physician, patient, health care team to determine appropriate timing of discharge from home health services.

    Performs procedures and treatments with proper technique and in compliance with standard nursing policy and procedure,within the scope of the nurse practice act. Seeks and maintains knowledge related to a variety of disease processes and related medications of patient population. Demonstrates proper use of equipment. Is a good steward of Mercy resources , manages time efficiently. Follows policy and procedure in regards to disbursement of supplies related to patient care, with attention to required authorization as determined by payer source.

    Documentation is accurate and completed within the expected timeframe. Documentation reflects knowledge of Oasis/Medicare/Medicaid regulatory guidelines. Corrections or amendments to documenation are made within requested timeframe. Assumes responsibility for continuing education to maintain current knowledge base. Completes annual mandatory education by date designated by Mercy Education/Compliance team. Participates in staff meetings, skills competency training, and Quality Improvement activities. Collaborates with fellow co-workers, including marketing team to evaluate and identify ways to improve Mercy Signature Service and ensure provision of excellent care.

    Provides supervision to the LPN and Nurse assistant as applicable. Assists with orientation of new co-workers. Participates in on-call, weekend/holiday rotation. Is flexible with daily schedule, able to prioritize. Displays positive attitude, despite challenging health care environment. Communicates regularly, utilizing professional and respectful behavior at all times both in verbal and written interactions with patients, families and co-workers. Compliant with Mercy's time and attendance policy.



    Position Qualifications:

    • We require a nursing degree from an accredited school.
    • You’ll also need a current RN license in our state.
    • Basic life support training should be completed in 90 days with advanced training required at certain intervals.

    One year experience in a related field.
