Under the general supervision of the Manager of Clinical Therapies and according to the standards of treatment, performs objectives assessments as well as comprehensive indiviualized treatment programs for patients of all ages, according to departmental policies and procedures regarding referral.
1. Perform initial assessment on each patient upon receiving referral according to
departmental standards of care.
1.1 Initiate initial treatment visit according to departmental standards.
1.2 Inquire as to patient's perceived goals of treatment of Physical Therapy.
1.3 Inform the patient of modes of achieving perceived goals courteously.
1.4 Perform appropriate objective tests with no errors according to departmental
policies and procedures regarding standards of treatment.
2. Document Initial Evaluation/ Plan of Care according to departmental policy.
2.1 Interpret objective tests to establish functional treatment goals with
defined time frames.
2.2 Initiate treatment procedures to achieve defined goals.
3. Administer appropriate modalities, exercise regimens, or treatment procedures
according to departmental policies and procedures on standards of treatment care.
3.1 Perform individualized treatment procedures in a professional and courteous
3.2 Reassess and update goals; modify treatment as indicated.
3.3 Utilize appropriate safety techniques and infection control methods
according to departmental policies and procedures.
3.4 Manage patient load according to productivity measures of the department.
3.5 Perform miscellaneous job duties as directed by the Manager of Clinical

repetitions, frequency, and precautions.
3.7 Educate family member or caregiver in rehab performance as well
as to patient's home procedures and precautions in a professional
and courteous manner.
4. Maintain progress notes and departmental records on each patient according to
departmental policies regarding documentation and charges.
4.1 Document interim and discharge notes according to departmental policies and

treatment procedures in a written legible progress note with consideration
of specified treatment goals. Document any changes in status or treatment
procedures on the day in which they occur. Notify physicians by written
correspondence or verbal communications as indicated.
4.3 Maintain a daily attendance record of each patient; record itemized patient
charges for services and equipment on a daily basis with 95% accuracy.
4.4 Document treatment plan/ progression for continuity of treatment among
therapists before scheduled day(s) off.
5. Participate in promoting education within the department.
5.1 Attend 80% of departmental inservice meetings/staff meetings.
5.2 Participate in required CQI activities according to established standards in

5.4 Maintain current CPR certification.
6. Demonstrates knowledge of special needs and behaviors of specific age groups to
include adolescent, adult and geriatric.

cultural, safety and other age related factors.
6.3 Demonstrates the ability to obtain and interpret information in terms of the

Education: Bachelor of Science in Physical Therapy.
Licensure: Current Oklahoma license of Physical therapy.
Certifications: American Heart Association (AHA) Basic Life Support required.
Preferred Experience: 2 years of Out-Patient Physical Therapy.
