Performs direct patient care activities for all age groups within the scope of practical nursing preparation under the direct supervision of an RN. The LPN’s practice must be consistent with the established guidelines of practice, the goals of Mercy, the Philosophy of the Sisters of Mercy, the Missouri Nurse Practice Act, the code of Professional and Business Ethics, and the Co-Worker Code of Conduct. Performs related duties as assigned.
Provides direct patient care under the direction and assistance of the RN.
1.1 Recognizes a life threatening/emergency situation, responds quickly, and notifies RN immediately.
1.2 Assesses patient’s ability to understand.
1.3 Interprets and utilizes diagnostic data.
1.4 Identifies psychosocial and spiritual needs.
1.5 Determines ability to perform activities of daily living.
Implementation of patient care under the direction and assistance of the RN.
2.1 Provides patient care according to written plan of care/clinical pathway.
2.2 Provides patient care according to established policies and procedures.
2.3 Utilizes critical thinking skills to meet the patient’s physical needs, as possible.
2.4 Utilizes critical thinking skills to meet the patient’s spiritual and psychosocial needs, as possible.
2.5 Demonstrates technical skills necessary to function in nursing practice.
2.6 Uses organizational skills and sets priorities in providing care.
2.7 Integrates teaching in all aspects of care.
2.8 Addresses call lights and alarms in a timely manner.
2.9 Reviews the plan of care with an RN.
Evaluation of patient care under the direction and assistance of the RN.
3.1 Evaluates patient’s progress in attaining expected outcomes. Notifies RN when patient is not progressing as expected.
Communication – First Touch
4.1 Utilizes “First Touch” communication techniques in all interactions.
4.2 Documents in a pertinent, thorough manner, consistent with established policies and procedures.
4.3 Keeps patient and authorized individuals informed on aspects of care throughout hospitalization.
4.4 Gives report in concise and pertinent manner using SBAR format.
4.5 Communicates necessary information to physician utilizing read-back when appropriate and follows-up as needed.
4.6 Keeps RN aware of abnormal findings not previously identified in patient and consults with shift coordinator RN regarding pertinent changes in patient conditions.
Demonstrates leadership skills within scope of LPN license.
5.1 Delegates tasks appropriately and effectively to nurse assistants/unit secretaries.
5.2 Follows up to see that delegated tasks are carried out as assigned.
5.3 Demonstrates patient advocacy skills.
6.1 Is compliant with bar code scanning.
6.2 Documents patient care according to written plan of care/clinical pathway and established policies/procedures.
6.3 Assures that Core Measures are implemented and documented per guidelines.
7.1 Recognizes patient safety needs.
7.2 Provides safe environment for patient.
7.3 Implements and documents fall precautions according to Fall Risk Assessment/Fall Prevention Policy.
7.4 Utilizes patient lift/transfer equipment appropriately and safely.
7.5 Wears proper Personal Protective Equipment when appropriate.
7.6 Observes Standard Precautions consistently.
7.7 Complies with Hand Hygiene Policy.
7.8 Completes appropriate reports regarding patient incidents.
8.1 Wears clothing and Health System badge consistent with dress code, and attends to personal hygiene to maintain a clean, well-groomed appearance.
8.2 Meets Health System attendance and punctuality standards.
8.3 Remains current with changes in policies/procedures and complies with policies/procedures.
8.4 Remains current by attending unit staff meetings/reading minutes.
8.5 Is responsive to staffing needs.
8.6 Performs additional duties as assigned.

Education: Graduate of a State Board of Nursing approved Practical Nursing Program.
Licensure: Current LPN licensure with the Missouri State Board of Nursing or current LPN Missouri temporary work permit.
Certifications: Must have a current American Heart Association Healthcare Provider card or complete a course within their probation period.
Other: Skills, Knowledge, and Abilities: Graduate of a generic practical nursing program after February 28, 1999 or successful completion of an IV Therapy Administration course approved by the Missouri State Board of Nursing. Working Conditions, Mental and Physical Requirements: Stand and walk most of the time on duty. Meet unit specific physical demands. Significant amount of bending and stooping, as well as lifting and turning patients with assistance. May be required to work differing schedules, shifts, days of the week, or work frequency according to patient care requirements. High risk exposure to blood and body fluids. Physical demands are attached plus push, pull, and grip up to 50 lbs., pinch up to 15 lbs., kneel, crouch, and crawl minimally. Equipment Used: Unit specific.
