Responsible for providing skilled nursing services to patients in their homes under the direction and supervision of a registered nurse. Must assess, plan, implement and evaluate patients needs and care. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.
Provides skilled nursing care to patients which includes physical assessment and assessing the patient mentally and physically on each visit and reporting to the supervising RN when appropriate.
Implements the plan of care as established by the RN and the interdisciplinary team and works toward those established goals. Evaluates effectiveness of the care plan and communicates frequently with the supervising RN regarding need for updating care plan.
Performs procedures and treatments with proper techniques and complies with standard nursing policies and procedures. Uses equipment properly to administer prescribed treatments. Practices cost containment with time and supplies.
Instructs and explains plan of care to patient and family, teaching as needed to encourage self care. Coordinates with additional services and community resources as appropriate. Plans and prepares patient and family for discharge.
Consults Physician, RN, patient, caregiver and other health care professionals
regarding coordination and treatment, goal development and response to treatment. Reports on above to RN and at patient care conferences. Identifies needs for other disciplines.
Charts medications, procedures and treatments accurately and completely. Documents care, condition, noticeable changes and vital signs completely, accurately and in a timely manner and instructions given. Completes all required paperwork within the required time and delivers to the appropriate office.
Assess home situation for possible patient abuse.
Participates in Quality Improvement activities, inservices, patient care conferences, workshops, and seminars. Assumes responsibility for continuing education and maintaining current knowledge.
Assist with orientation and training of other staff members.

IV certification required.
Possesses and maintains current CPR certification thru the American Heart Association.
