    Responsibilities and Qualifications

    Mercy MS Care is a brand-new Multiple Sclerosis Center serving over 2000 MS patients in the St. Louis area.


    Mercy MS Care is a department of Mercy Hospital St. Louis and is modeled after some of the most successful MS Centers in the country. Working in partnership with Mercy Clinic Neurology, Mercy MS Care provides patients with a dynamic care management approach, addressing 600 patient advocacy and clinical issues per month.


    The Social Worker II reports to the Director of Mercy MS Care. The position is a lead role and an intrinsic part of the growth of the organization. Providing social services, leading support groups and individual therapy sessions the Social Worker II is at the heart of Mercy MS Care. The role strikes a balance between billable hours and non-funded services.


    The ideal candidate is an organized, self-starter with an eye for detail.  They will have knowledge of MS and chronic illness or have a willingness to learn. The Social Worker II will become the subject matter expert on social services available in St. Louis for MS patients. They will have a professional presentation and a warm outgoing personality which will enable them to connect with all races and economic levels. The ideal candidate will identify strongly with the Mercy mission and have a love for all people.


    The Social Worker II, as part of the Care Management team, intervenes with patients who have complex or high-risk psychosocial needs. In addition, the Social Worker II offers crisis intervention to patients and families and coordinates and facilitates free support programs and services. This role receives patient referrals from team members (including physicians, Care Managers, staff nurses and other members of the care team). In addition, the Social Worker II will build a client base by interfacing with patients as they frequent the clinic for care, building relationships of trust with the patients and educating them on the services and mission of Mercy MS Care. The Social Worker II performs duties and responsibilities in a manner consistent with the Mercy mission, values, and Service Standards.

    • Complete psychosocial assessments and interventions with high-risk, complex patients and families.
    • Intervene with patients and families regarding emotional, social, and financial consequences of illness/disability and mobilize resources to meet needs.
    • Provide case intervention for adult abuse/neglect, domestic violence, sexual assault.
    • Provide resources and education while advocating for patient empowerment to make autonomous healthcare decisions and access needed services within the healthcare system and the greater St. Louis area.
    • Support Care Management plans and processes to measure, assess and improve quality patient outcomes and Care Management metrics.
    • Demonstrate ethical behavior and provide confidentiality in all Care Management interactions.
    • Maintain timely documentation of all patient interactions including assessment and interventions.
    • Portray a positive professional public image at all times.
    • Promote individual growth and accountability by meeting requirements for mandatory/continuing education and skills competency.
    • Serve as preceptor, mentor, and social work resource to other co-workers at Mercy.
    • Work with and manage social service volunteers
    • Demonstrate Mercy mission, values and charism.
    • Perform other duties as assigned.

    Education: Masters in Social Work required for all Social Workers hired after 12/25/2011.

    (Social Workers hired before December 2011 met previous job description education requirements.)


    Licensure:Current license in Social Work, master’s level or above, in the state of employment required. LCSW preferred


    Experience: 0-2 years experience in acute care hospital setting.

    Individual therapy and group facilitation experience.


    Certification/Registration: BLS (CPR) at hire date, required, or within 90 days of hire


    Other Skills & Knowledge

    - Strong verbal and written communication and facilitation skills.

    - Strong assessment skills, time management skills, critical thinking skills.

    - Advanced computer skills; advanced Excel and Outlook preferred.

    - Skills and knowledge of human behavior and clinical aspects of medical Social Work.

    - Ability to interact effectively with a variety of people and situations at all levels of the organization.
