    Position Qualifications:


    Licensure: Current Oklahoma RN License
    Certifications: Current BLS Certification required before assigned to care for patients independently
    Other: Demonstrates competence by completion of Universal Skills Inventory and Unit Specific Skills Check List within first three months of employment and undated annually.
    Preferred Certifications: CHPN

    Responsibilities and Qualifications

    Assesses the hospice patient's condition, environment, safety as well as patient and
    family needs in aconsistent and systematic manner.
    1.1 Reviews history, previous
    notes, and effectiveness of interventions towards meeting goals of hospice patient and
    or family every visit.
    1.2 Utilizing input of all sources of information, collaborates with various members of
    the hospice IDG to ensure the best care for the patient.
    1.3 Completes the clinical, psychosocial and environmental assessment as
    appropriate related to the care, treatment and provision of services to the terminally ill
    1.4 Identifies high risk bereavement and other barriers during the care of the terminally
    ill patient.
    1.5 Demonstrates the ability to obtain and interpret information in terms of the patient's
    1.6 Demonstrates an excellent understanding of disease process, pain and symptom
    management in day to day practice to ensure the utmost in quality of care for patient
    and family.
    1.7 Demonstrates an excellent understanding of death and dying and promotes
    awareness and understanding to patient and or family.
    1.8 Demonstrates an excellent understanding of pharmacology and its application to
    the terminally ill patient in promoting comfort and quality of life. Able to perform
    conversions of opiods if indicated to improve quality of life.
    1.9 Demonstrates an excellent understanding about home safety, infection control and
    educates the family and patient in the prevention of falls and fires and other safety
    Develops the ongoing, outcome-oriented plan of care for assigned group of primary patients. Plan of care reflects the patient's actual or potential health related problems.
    2.1 Based on assessment, involves patient and caregiver (s) in identification
    and ongoing decision making regarding goals and approaches to care.
    2.2 Prioritizes care delivery based on the patient and or family needs.
    2.3 Collaborates with other health care providers and caregivers to modify the plan of care as needed based on the needs of the changing hospice patient and or
    family needs.
    2.4 Demonstrates ability to anticipate patient/family needs and potential problems
    based proactive, thorough assessments.
    2.5 Documents revision of plan of care as indicated, and at least every two weeks.
    2.6 Documents changes in the level of care based on the changing needs of the
    2.7 Performs recertifications in accordance to NHPCO and medicare guidelines.
    Implements and coordinates nursing care to assist the assigned primary patients and
    family to achieve their mutually identified individual goals.
    3.1 Provides
    professional nursing care to assigned home care patients according to the plan of care.
    3.2 Instructs patient, family and caregivers to achieve competence and confidence in
    provision of independent care.
    3.3 Identifies and implements safe, economical and effective alternatives to achieve
    3.4 Ensures continuity of care by taking responsibility for identifying and
    communicating all pertinent information related to patient's ongoing health care.
    Evaluates the effectiveness of all interventions provided or facilitated by the home care
    interdisciplinary team.
    4.1 Evaluates patient/family responses to care, determines
    progress towards achievement of outcomes at each visit, and adjusts plan of care
    4.2 Evaluates appropriateness of current level of care and recommends appropriate
    changes in frequency.
    4.3 Facilitates referrals to other disciplines or levels of care and as indicated.
    4.4 Performs supervisory visits on per visit RN's, LPN's, and HHA's according to
    departmental guidelines.
    Demonstrates accurate and effective verbal, written and electronic communication
    within departmental timeframes.
    5.1 Creates a positive climate for communications
    with patients, families and colleagues.
    5.2 Maintains confidentiality of patient, personnel and organizational information.
    5.3 Demonstrates constructive problem-solving techniques when interacting with
    patients, families and co-workers.
    5.4 Demonstrates competency in utilization of departmental and organizational
    information systems.
    5.5 Completes all required documentation according to departmental guidelines 95%
    of the time.
    Demonstrates professional conduct and evaluates his/her nursing performance based
    on accepted professional standards of practice..
    6.1 Integrates research-based,
    current information to enhance or improve clinical practice by proactive problem
    6.2 Contributes to agency growth and development by actively participating in
    departmental and organizational meetings, committees, task forces and special
    6.3 Serves as a role model and preceptor for new staff members, students or visitors.
    6.4 Demonstrates knowledge of needs and behaviors of special needs and behaviors
    of specific age groups to include: neonatal, pediatric, adult and geriatric.
    6.5 Completes all mandatory educational activities.

    achievement of goals at least annually.
    6.7 Completes competency checklist during orientation and at least annually.

    7.1 Demonstrates flexibility in schedule, based on department needs.
    7.2 Participates in weekend and call rotation per departmental policy.
    7.3 Demonstrates awareness of costs related to patient care. Practices good judgement in preventing waste. Organizes and prioritizes assignments to complete them within allotted time.
    7.4 Assists in the care of equipment and supplies to ensure availability when needed.
    7.5 Participates in departmental performance improvement activities by communicating opportunities for improvement and customer concerns and by assisting with specific projects as requested.
    7.6 Participates in risk management activities by completing incident reports and problem identification report by the close of the business day.
    7.7 Provides a safe environment for patients, visitors and staff.
    7.8 Performs miscellaneous job duties as assigned.
