    Responsibilities and Qualifications

    Under the direction of an RN, the Licensed Practical Nurse provides nursing care to patients in the assigned area. This includes providing care in conjunction with therapies as directed by other disciplines. Assists patients with meals and daily care. Administers medication and treatment as prescribed by physician. Transcribes physician's orders and documents on nursing PCP. Performs duties and responsibilities in a manner consistent with our mission, values, guiding principles and Mercy Standards.


    Safety/Risk Management
    a. Adheres to counting policy by administering all substances, supplies and instruments pertaining to patient care, including, but not limited to:
    - Instruments
    - Narcotics
    - Sponges
    b. Follows policy/procedure for Incident Report/Sentinel Events
    c. Follows Confidentiality Policy/Procedures.
    d. Complies with Infection Control Guidelines.
    e. Creates a safe environment by adhering to safety goal programs and policies, including, but not limited to:
    - Assessing for and implementing Fall Precaution Program
    - Restraint Management
    - Utilization of Patient I.D. identifiers
    - Reading back of telephone and verbal orders; sign off of daily orders
    - Time out procedures
    - Communication hand offs
    - Medication safety practices; barcode compliance and medication reconciliation
    - Awareness of dangerous abbreviation and questioning whenever used by any member of HCT
    - Utilizing braden score to prioritize patient risk for skin care interventions and the prevention of Nosocomial wounds
    - Unit/departmental specific policies
    f. Respects patient/guest’s personal property.
    g. Adheres to hospital wide and specific department safety regulations, including, but not limited to::
    - Fire and Tornado
    - Environmental Safety
    h. Performs equipment safety checks.
    i. Complies with patients rights/regulations.

    Coordination of Services
    a. Coordinates workflow by communicating to appropriate personnel in a timely manner.
    b. Demonstrates ability to assist with the work activities of department consistent with patient needs and staff capabilities.
    c. Participates in quality improvement activities that ensure and enhance quality of patient care and/or
    reporting mechanisms.
    d. Transcribes physician’s orders accurately, seeking clarification as necessary.
    e. Participates in unit’s formal QI Program.
    f. Actively participates in unit activities, committees, teams which promote unit operations.


    Primary Aspects
    a. Demonstrates proper time management skills and priority setting.
    b. Participates in continuing patient assessment, identifies actual physical, social, psychosocial and
    spiritual needs of patient and families, and documents in the medical record.
    c. Observes changes or unusual occurrences in the patient’s condition and reports them immediately to the appropriate personnel.
    d. Initiates and utilizes care paths appropriately.
    e. Utilizes appropriate resource person in the implementation of plan of care. Documents evaluation
    of effectiveness.
    f. Provides input for and utilizes the established plan of care. Reviews care plan for optimum patient benefit. Participates in patient care conferences.
    g. Assists in the implementation of established patient teaching plan. Utilizes resources.
    h. Maintains and updates knowledge of current nursing practice. Seeks appropriate learning experiences.
    i. Demonstrates appropriate knowledge base related to medications and adheres to hospital policies
    regarding their administration.
    j. Assures all patient charges/credits are entered appropriately.
    k. Documentation is factual, accurate, organized, complete, current, and confidential.
    l. Demonstrates flexibility regarding workload, assignments and change.

    Communication Management
    a. Communicates patient goals, objectives and changes in status to other health care team members and utilizes input to revise care plan for optimum patient benefit.
    b. Shift report is factual, accurate, organized, complete, current, and confidential.

    a. Demonstrates competence in initiating and maintaining use of routine equipment and procedures.
    b. Demonstrates competence in assisting physicians with diagnostic and therapeutic interventions.
    c. Performs technical skills relative to area of nursing practice.
    d. Assists in evaluation of new products, equipment, policies, procedures and standards.

    Licensure: Must have either a current Licensed Practical Nurse Temporary Permit/LPN license in the State of Missouri
    Certifications: Must be certified in IV therapy.
