    Responsibilities and Qualifications

    Demonstrates effecive verbal and written communication
    1.1 Answers telephone in unit appropriately and courteously, identifying self with name and title. Able to facilitate call to appropriate person or take a message.
    1.2 Prioritize all calls from physicians, patients, and families and support areas and relays information received to appropriate person within 5 minutes.
    1.3 Traffics medical personnel, patients and families in a professional courteous manner to proper locations.
    1.4 Maintains patient confidentiality according to HIPAA guidelines and hospital policy.
    1.5 Communicates audio alerts from surveillance systems and any exceptional behavior/activity of patients to assigned nursing personnel immediately upon observation.
    1.6 Promptly reports all signs and symptoms indicating changes in client condition to appropriate nurse.
    1.7 Reports to appropriate nurse any patient needs/responses to therapy (complaints of pain, response to surroundings, changes in alertness, unusual skin redness or tears, etc.).
    1.9 Reports off to charge nurse and oncoming assistant before leaving assigned area.
    1.10 Documentation of care and other pertinent data is completed according to organizational policies and procedures.
    1.11 Creates a positive climate for communications
    1.12 Assists with promptly answering of call lights

    Operate computer/office machines/monitors, according to procedure.
    2.1 Operate imprinters, telephones, copy machines and emergency equipment according to procedure.
    2.2 Retrieve information from computer immediately when requested by physicians, nurses or approved personnel according to procedure.
    2.3 Operate nurse call system according to procedure.
    2.4 Operate intercom system according to procedure.
    2.5 Operate cardiac monitors according to policy.
    2.6 Run and post strips every 4 hours and as needed according to patient status.
    2.7 Process all patient charges.
    2.8 Demonstrates the ability to obtain and interpret information in terms understandable to the adult and geriatric patient.

    Has knowledge and understanding of daily assignments and unit activities.
    3.1 Answers patients' call lights in a timely manner to determine needs and responds appropriately by fulfilling needs within the scope of job description or reports to the nurse.
    3.2 Seeks clarification of tasks as indicated.

    Demonstrate appropriate office procedure.
    4.1 Transcribe physician's orders into computer according to procedure with no errors.
    4.2 Transcribes physicians orders by using "Downtime Procedure" when computer is down.
    4.3 Sends physicians orders carbons to pharmacy for patient's medications
    4.4 Post patient's test results and cares notes in chart within two hours of receipt according to procedure.
    4.5 Initial and sign-off orders as entered into computer.
    4.6 Disassemble entire patient chart after dismissal and send to medical records.
    4.7 Maintain patient census accurately according to procedure with no errors.
    4.8 Assemble patient chart within 30 minutes of admission.
    4.9 Re-stock unit secretary area and patient rooms daily.
    4.10 Receives and delivers lab specimens according to lab or department procedure.
    4.11 Observes and expedites work coming through the USC desk within 30 minutes or less of receipt according to procedure.

    Performs routine tasks and simple treatments after completing task competencies check list.
    5.1 Performs fingerstick blood sugars, records results in medical record, and reports abnormal results (less than 70 or greater than 120) to the nurse.
    5.2 Properly disposes of suction containers at 0600 each day. Sets up replacement.
    5.3 Takes and records temperature on all assigned patients every 4 hours or as directed by nurse. Reports any significant findings to the nurse.
    5.4 Empties or changes wound suction/drainage devices at 0600, 1400, 2200 and prn; records amount on graphic record, if task requested from RN.
    5.5 Provides heat lamps, applies and refills ice packs, refills K-pad units and humidifiers, and may measure for and apply TED hose.
    5.6 Obtain daily weight using Hill rom bedscale and document on daily flowsheet.
    5.7 Turns and encourages deep breathing and coughing by post-operative patients every two hours.
    5.8 Applies pneumatic compression stockings.

    Provides personal hygiene and comfort measures as directed and documents on daily assignment sheet.
    6.1 Provides complete/partial baths or showers, including oral hygiene (to include dental care).
    6.2 Provides fresh ice water at least once per shift.
    6.3 Change linens, making an occupied bed and unoccupied bed.
    6.4 Provides back rub, proper body alignment and turning of client with multiple lines and tubes during hygiene.
    6.5 Shave a male client, provide hair care, and nail care.
    6.6 Documents hygiene and comfort measures on flow sheet.
    6.7 Assists with bedpan/urinal, bedside commode, or to the bathroom as needed.
    6.8 Cleans and empties ostomy bags as needed.
    6.9 Performs post-mortem care when instructed.
    6.10 Perform perineal care/catheter care.
    6.11 Measure/record I&O on flowsheet and report to appropriate nurse when directed.

    Assist in client activity.
    7.1 Maintains proper body alignment and activity by turning non-ambulatory patients every two hours and documenting position on flow sheet.
    7.2 Assist a client to raise head of bed as directed.
    7.3 Use of proper body mechanics to prevent injury to self and patient.
    7.4 Transfer of dependent client from bed to stretcher.
    7.5 Position a client on his side and dangle.
    7.6 Help transfer client from bed to chair to wheelchair.
    7.7 Assist client with ambulation.

    Assists in the maintenance of adequate nutritional state.
    8.1 Serves between meals supplements/snacks as ordered.
    8.2 Serves trays to client when directed by nurse.
    8.3 Assists patients with meals, feeding if necessary.
    8.4 Collects trays after meals, recording percentages of food and liquids served on a daily flowsheet.
    8.5 Places trays on cart/disposes of trays per hospital policy.

    Maintains a clean and safe environment for patient and self
    9.1 Emptying and properly bagging linen and trash and storing it according to hospital policy.
    9.2 Cleaning bedside and overbed tables and straightening/changing bed linen prn.
    9.3 Removing unnecessary equipment from room.
    9.4 Uses proper hand washing techniques and uses standard precautions at all times.
    9.5 Cleaning and properly storing bedpans, urinals and bedside commodes.
    9.6 Complies with isolation procedures.
    9.7 Keeps bed at low position, call light within patient's reach, and alerts patient and staff to fall risk.
    9.8 Complies with dress code standards.
    9.9 Checks and records medication refrigerator temperature.
    9.10 Obtains supplies from clean holding.

    Promotes professional and organizational development
    10.1 Submits a completed skills checklist to Nurse Manager within the first three months of employment and annually thereafter.
    10.2 Participates in orientation of new employees.
    10.3 Attends mandatory inservices and/or completes mandatory lap packets in a timely manner.
    10.4 Obtains and maintains CPR.
    10.5 Attends mandatory inservices and 75% of staff meetings.
    10.6 Maintains current Fingerstick Blood Sugar certification.
    10.7 Maintains competency in basic dysrthymia recognition.
    10.8 Maintains competency in medical terminology.

    Education: High School Diploma or equivalent
    Certifications: BLS, certified. Required certifications must be current before assigned to care for patients independently.
    Other: Must be familiar with procedures involved in patient tasks. Completes orientation through Educational Resources before beginning clinical orientation on the nursing unit.
    Preferred Experience: Previous hospital experience.
    Preferred Certifications: Must have current certification for CNA from state of Oklahoma or 1 semester of nursing school. Completion of medical terminology. Basic Dysrhythmia.
