    Responsibilities and Qualifications

    Provides a professional clinical component to the Care Coordination process at Mercy. The primary function involves a collaborative relationship with RN and Social Work Care Coordinators in assessment, planning, coordinating, facilitating and implementing a quality cost effective plan that meets or exceeds the needs of the patient and his/her family. The scope of the plan will include patient advocacy, case facilitation, crisis intervention, discharge planning, patient, staff and physician education, quality and process improvement and utilization management. Performs duties and responsibilities in a manner consistent with our mission, values, and Mercy Service Standards.




    Because you’re committed to excellence, you understand the importance of being properly prepared for your role at Mercy. That’s why you’ll bring to your role the right set of qualifications:



    Education: Bachelors degree required

    Licensure: RN license in the State of MO required.

    Experience: Recent acute bedside experience within the past 3 years or

    current cases management experience required.

    Certification/Registration: CPR required, other certifications as determined by department

    Other skills & knowledge:

    Must possess strong oral and written communication skills, assessment/interview skills, organization skills, critical thinking skills, analytical skills, problem-solving skills, interpersonal skills, and listening skills. Must be fflexibility and be able to quickly adapt to change.






    Certification/Registration: Certification in Care Coordination preferred.
