Provides direct patient care to all age groups under the direct supervision of a Registered Nurse consistent with the established guidelines of practice, the Goals of Mercy's Health System, the Philosophy of the Sisters of Mercy, the Code of Professional and Business Ethics and the Employee Code of Conduct.
Receives on-coming shift report from nurse in a timely fashion.
Reports patients progress and unusual findings to nurse in a timely manner.
Assists patient with body hygiene.
Provides skin care.
Provides body/oral hygiene.
Dangling and walking
Sitting in chair
Bathroom privileges or commode
Using bedpan or urinal
Other special activities
Straightens and changes linen in occupied and unoccupied beds.
Keeps patient rooms clean and in good order.
Provides for needed supplies/items for patient comfort.
Answers call lights promptly.
Ensures that menus are obtained completed, collected and returned.
Ensures that patients nourishments are correct.
Assures that patients have correct trays.
Prepares patients for meals.
Sets up food trays, serves, collects, and returns them to cart.
Feeds or assists in feeding patients, as necessary.
Observes and records patients appetites.
Obtains and serves late trays, additional food, special nourishment and fresh water as necessary.
Records weights and graphics correctly.
Records activity on ADL flow sheet.
Records I&O correctly when indicated.
Assists with dismissal of patient.
Prevents skin breakdown by repositioning patient every two hours, if indicated.
Obtains accurate vital signs (BP, respirations, apical and radial pulse, temperature).

Education: Successful completion of a recognized Nurse Assistant training course.
Certifications: Certified Nurse Assistant BLS Certification required within 90 days of hire.
